Inquiry Module Reports


Inquiry reports are evidence-based documents that draw on the large amount of evidence gathered during the Inquiry’s investigations. The reports may contain references to death, illness, harm and suffering. You can find information about support services on the Inquiry’s support page.

The Inquiry published its first report and recommendations following its investigation into the UK’s ‘Resilience and preparedness (Module 1)’ on Thursday 18 July 2024.

It examines the state of the UK’s central structures and procedures for pandemic emergency preparedness, resilience and response.


The Chair of the Inquiry, The Rt Hon the Baroness Hallett DBE set out her recommendations from the Module 1 report in a live streamed statement now available as a recording on the Inquiry’s YouTube channel.

The Inquiry published its second report and recommendations following its investigation into the ‘Core decision-making and political governance’ on Thursday 20 November 2025.

It looked into core political and administrative governance and decision-making. It includes initial response, central government decision making, political and civil service performance as well as the effectiveness of relationships with governments in the devolved administrations and local and voluntary sectors.
